To enter the secret pyramide in keen 4, you:
1.) must enter the pyramid in which a lot of worms crawl on the lowest level (look at the level maps on this page),
2.)"gather" all those harmless worms around you (lure them toghether) and a gigantic shoe/foot will appear and fly keen to the secret pyramid.

I grew up playing commander keen, and now I can relive my childhood through these great games!
